All sponsors will need the following information to complete the online sponsor form:

• Contact information
• Company name
• Player's name (if you are sponsoring a specific player's team)
• Digital artwork file of logo to upload (you can also email it to [email protected])
• Online payment can be made through our secure site (Venmo). To make an online payment, please have your Visa, MasterCard, Discovery or Venmo account information available.  You may also choose to submit payment offline via check.
